PA-824 (Pretomanid) is a promising nitroimidazole drug with potent activity against Mycobacterium tuberculosis. In vitro, PA-824 demonstrates high efficacy against both drug-sensitive and multidrug-resistant (MDR) strains of M. tuberculosis, with MIC values typically below 1 μg/ml. Its activity spans clinical isolates from diverse geographic regions, including Asia (India and South Korea) and the United States, with MICs ranging between 0.015 and 0.25 μg/ml. [1]

A study investigating resistance mechanisms revealed that mutations in PA-824 resistance-associated genes, such as fgd1 (Rv0407) and ddn (Rv3547), do not significantly impact its MICs, suggesting its robustness against certain resistance mutations (MICs ≤ 0.25 μg/ml). [2]

Moreover, PA-824 has been shown to exert bactericidal activity under hypoxic conditions, targeting both replicating and non-replicating M. tuberculosis populations. This unique dual-phase activity further emphasizes its potential in treating both active and latent tuberculosis infections. [3]
