Aldose Reductase

Aldose reductase inhibitors block the first enzyme of the polyol pathway, which reduces glucose to sorbitol. Aldose reductase is encoded by aldo-keto reductase family 1 member B1 (AKR1B1) and uses nicotinamide adenine dinucleotide phosphate (NADPH) as cofactor. Under high glucose, flux through the polyol pathway rises, a mechanism reviewed in 2001 as one route to diabetic complications. A 1992 structure of the human holoenzyme at 1.65 Å resolution revealed an unexpected sugar substrate site. Epalrestat and tolrestat carry carboxylic acid groups, whereas sorbinil and fidarestat are spirohydantoins, and ranirestat and imirestat are further spiro-fused inhibitors. AT-007 is a brain-penetrant inhibitor studied in galactose metabolism models.
